Enhanced Care Management
Receive the Care You Need
Enhanced Care Management offers no-cost services to managed health plan
members get the health care and local services you may need. When you
join this program, you will have a dedicated care team to support you
as you work toward your health goals.
What is Enhanced Care Management?
This is a no-cost program for managed health plan members with complex
health issues or social needs. If you qualify, a care team will provide
services to help address your individual needs, these services include
assistance with:
- Physical health
- Behavioral health
- Substance use treatment
- Social services
Your care team can also help you coordinate:
- Find providers
- Follow-ups
- Medications management
- Post hospitalization services
- Transportation
How can the program help you?
Received your own care team, which includes your Doctor, Nurse Services
Care Manager, and Care Coordinator. Your care team works with all your
doctors and your personal support system to create a special care plan
just for you.
Your care team will help you:
- Keep all your doctors fully informed
- Set goals for getting and staying healthy
- Find and apply for county and community services, like food, housing, jobs, childcare, legal assistance and more.
Support when you need it
Your care team can support you by phone or in person at an office near
you. You’ll never feel alone with Enhanced Care Management. Over
the last three years, this program has helped more than 500 community
members with their health care needs and has reduced hospital admissions by 90%.
Get more services
Enhanced Care Management will not change any current plan services you
have. You can keep your current Doctor and Specialists, plus you will receive:
- Your own care team
- A special care plan just for you
- Guidance through the health care process
Who is eligible?
You may be eligible for ECM if you are experiencing any of the items below:
- Unstable housing
- Have multiple health issues and have needed to go to the emergency department or have been hospitalized many times over the past six months
- In need of mental health services
- Struggle with drug or alcohol use
- Released from jail or prison and need help transitioning to the community
These services are at no cost; you may join or stop the program at any time. We will not change your primary doctor. We will work with your own provider.
